OPTIONS FOR THE SUBMISSION OF THE APPLICATION TO SUBSCRIBE

1. Via any PVB branch. Accomplish the form  and manually submit to any PVB branch together

with your payment. Please bring any government issued ID for identification purposes.

2. Via online application. Click on the link below to apply online.

PAYMENT INSTRUCTIONS:

The payment for the subscription price must be received by the Bank, through any of the payment options below, in full, without any deduction.
All bank charges shall be for the account of the Subscriber.

The Bank reserves the right to reject an Application or reduce the number of Subscription Shares in the
following and analogous cases:

(a) failure to submit the Application with the required payment and other requirements within the Offer Period,

(b) failure to comply with the procedures and requirements for the Subscription Offer, or

(c) failure to subscribe for the Shares.
