Borrower Qualifications:

· Successfully booked Salary Loan during the promo period

· Minimum Loan Amount of Php 100,000

· Minimum Loan Term of 24 months

Promo Mechanics, Terms and Conditions:

1. All successfully booked Salary Loans during the promo period with a minimum loan amount of Php 100,000 and a loan term of 24 months automatically receive the complimentary Medicare Plus ER Guard Plan (Emergency HMO Coverage).

2. Upon release of net proceeds of the Salary Loan to the borrower, he/she will receive the reference number and security code to activate the HMO benefit via the Medicare Plus website (https://shop.medicareplusinc.com/).

3. The HMO benefit shall be at no cost to the borrower and will be a value-added feature of the Veterans Bank Salary Loan.

4. Activation and utilization of the HMO coverage shall adhere to Medicare Plus's terms and conditions and prevailing rules and procedures as follows:

MEDICARE PLUS ER GUARD TERMS AND CONDITIONS

· Valid for one (1) year from activation.

· One-time emergency use only.

· Non-transferable once activated.

· Damaged or tampered cards are invalid.

· Must follow Medicare Plus rules and limitations.

Emergency Coverage Inclusions:

The Medicare Plus ER Guard plan provides up to Php 20,000 worth of emergency medical treatment. Nationwide access to all private hospitals and providers including:

· Physician's Services: consultations and professional fees during emergency care

· Emergency Room Fees: charges related to ER use for emergency treatment

· Emergency Medicines: Medicines used for immediate relief and treatment during the emergency visit

· Oxygen, Intravenous (IV) Fluids, Blood Products: covers necessary stabilizing treatments (except gamma globulin and immunoglobulin)

· Medical Supplies: dressings, plaster casts, sutures and other supplies needed for emergency care

· Diagnostics Related to the Emergency: includes laboratory tests, x-rays, and other diagnostic exams directly related to managing the emergency condition

· Initial Treatment of Animal Bites: includes first 24-hour initial treatment; passive and active vaccines for anti-tetanus, anti-venom and anti-rabies; and vaccine cost up to P2,500 for first dose only.

5. Borrower may transfer the HMO benefit to their dependent by enrolling their dependent instead of themselves. Once activated, the HMO benefit becomes non-transferable.

6. The value of the HMO coverage is not convertible cash, gift voucher or certificate.

7. Get a PVB Bank Salary Loan with Free Emergency HMO Coverage Promo is subject to terms and conditions governing Veterans Bank's Salary Loans. All decisions of the Bank regarding this promo, in concurrence with DTI rules and regulations, will be deemed final and non-negotiable.

8. Employees of Philippine Veterans Bank, its subsidiaries and outsourced agencies, including their relatives up to second degree of consanguinity or affinity are not eligible to avail of the promo.
